Madagascar IVF: Cost, Laws & Embryo, Sperm, Egg Transport for Fertility Treatment

Introduction
Expert reviewed by Sarah Adekola, BSc Biomedical Science (First Class Honours)- Human Anatomy Major, Certificate in Clinical Embryology and Andrology
IVF is available in Madagascar, but only in a small number of private clinics, almost all in Antananarivo. That is why so many Malagasy couples, and the large Malagasy community in France and elsewhere, need to move sperm, embryos or eggs into or out of the country during treatment.
One of the most common needs is sperm transport. A husband working abroad can freeze his sperm where he lives and have it hand-carried to the clinic in Antananarivo, so treatment goes ahead without him flying home.
This guide covers everything you need to know: where IVF is offered, what the law says, how much treatment costs, and how to transport your embryos, sperm and eggs safely in and out of Madagascar. We are Cryo Medical Logistics, and we manage supervised, hand-carried cryogenic transport of reproductive material across 80+ countries.
TL;DR
Is IVF available in Madagascar? Yes. A small number of private clinics offer IVF and ICSI, concentrated in Antananarivo.
Can I send sperm to Madagascar? Yes. Frozen sperm can be hand-carried from anywhere in the world to a clinic in Antananarivo, ideal when a partner lives or works abroad. Sperm can also be shipped out of Madagascar for ICSI, testing or storage.
Can I ship embryos or eggs to or from Madagascar? Generally yes, for your own material going to a licensed clinic. There is no specific ban, but paperwork is handled case by case, so professional coordination matters.
Is IVF legal? Yes. Madagascar has no dedicated IVF law. Treatment is generally offered to married heterosexual couples using their own eggs and sperm.
How much does IVF cost? Around 20 million ariary per cycle has been reported locally (roughly US$4,000–5,000), before medication and extras.
What is the safest way to transport sperm, embryos and eggs? A supervised, hand-carried courier with a −196°C dry shipper, a Do Not X-Ray protocol and full chain-of-custody records, never unaccompanied cargo.

Where can I get IVF in Madagascar?
IVF in Madagascar is offered by a small but growing number of private fertility centres in Antananarivo. Local press reported in 2022 that fertility centres were multiplying in the capital, with new providers offering IVF, extended embryo culture to the blastocyst stage and frozen embryo transfer.
Outside Antananarivo, we are not aware of any IVF provision. Patients from Toamasina, Mahajanga, Fianarantsoa, Antsiranana and Toliara usually travel to the capital, or abroad to Mauritius, Réunion or mainland France.
Services typically available in Antananarivo include fertility assessment, IUI, IVF, ICSI, embryo freezing and frozen embryo transfer. Our vetted partner clinic network can advise on the options that suit your case.
How does Madagascar compare with West Africa's fertility sector?
Country | IVF availability | Main IVF cities | Dedicated IVF law | Who is typically treated | Embryo, sperm and egg transport |
Madagascar | Small, growing private sector | Antananarivo | No | Mainly married heterosexual couples, own gametes | Possible case by case; no specific ban |
Nigeria | Largest in the region; 170+ clinics | Lagos, Abuja, Port Harcourt, Ibadan, Enugu, Benin City, Kano | No | Couples; donor eggs and sperm widely used | Well established, frequent inbound and outbound shipments |
Ghana | Well established | Accra, Kumasi, Takoradi | No comprehensive law | Couples; donor gametes available | Well established |
Côte d'Ivoire | Established, several centres | Abidjan | No | Mainly married couples | Possible case by case |
Senegal | Established, several centres | Dakar | No | Married couples, own gametes | Possible case by case; donor material sensitive |
Madagascar's sector is closest in size and structure to Francophone West Africa: a handful of private centres in one city, no dedicated IVF legislation, and treatment focused on married couples.
Is IVF legal in Madagascar, and who can have it?
Yes, IVF is legal in Madagascar, but there is no dedicated law regulating it. Madagascar has a general law on reproductive health and family planning, which does not set detailed rules for IVF or for moving reproductive material across borders.
In practice, this means:
Married heterosexual couples using their own eggs and sperm are the standard patient group and face the fewest obstacles.
Donor eggs and donor sperm are not specifically regulated. Availability depends on the clinic, so confirm before you plan treatment.
Single women and same-sex couples have no clear legal pathway and are rarely treated.
Surrogacy is not regulated. Under Madagascar's civil law, the woman who gives birth is generally treated as the legal mother, so surrogacy carries real parentage risk. Take independent legal advice before considering it.
Because the rules are set more by clinic practice than by statute, your receiving clinic's written agreement is the most important document in any treatment or transport plan.
How much does IVF cost in Madagascar?
A single IVF cycle in Madagascar has been reported at around 20 million ariary, roughly US$4,000–5,000, with some newer providers aiming to price below tha. Medication, testing, freezing, storage and transport are usually charged on top.
How do I send sperm to Madagascar for IVF?
Frozen sperm can be shipped to a Malagasy clinic once the clinic confirms in writing that it will receive and store it. This is common when a husband works abroad and cannot be in Antananarivo on the day of treatment. The sperm is frozen at a clinic where he lives, then hand-carried to Madagascar in a −196°C dry shipper.
Sperm can also be shipped out of Madagascar, for example to a clinic abroad for ICSI or for long-term storage.

Can I ship frozen embryos to or from Madagascar?
Yes. There is no specific law banning the import or export of embryos, so shipping your own embryos between licensed clinics is generally possible. Because no single authority has a fixed procedure, each shipment is cleared case by case with the receiving clinic, the health authorities and customs.
Common reasons people ship embryos to or from Madagascar include:
Returning home to Madagascar with embryos created in France, Mauritius, South Africa or elsewhere
Sending embryos created in Antananarivo abroad for genetic testing (PGT) or treatment
Relocating stored embryos when moving country
Moving embryos between clinics for a better treatment option
Can I transport my frozen eggs to Madagascar?
Yes, if your receiving clinic agrees to accept and use them. Frozen eggs are delicate and must stay at −196°C throughout the journey. Many women freeze eggs in Europe or Mauritius and later bring them home to Madagascar for treatment.
Donor eggs and donor sperm are more sensitive. Your Malagasy clinic must confirm in writing that it accepts donor material before anything is shipped.
How long does transport to or from Madagascar take?
Most international shipments take two to three days door to door. There are no direct flights between Madagascar and the UK or USA, so every shipment connects through an international hub. Our courier stays with your material at every stage, and the dry shipper holds −196°C for several days beyond the planned journey as a safety margin.
Route | Why people use it | Typical door-to-door time |
France ↔ Madagascar | Diaspora couples bringing sperm or embryos home, or treating in France | 2–3 days |
UK ↔ Madagascar | Sperm or embryos frozen at UK clinics; partners working in the UK | 2–3 days |
USA ↔ Madagascar | Donor eggs and sperm, embryos created in the USA | 3–4 days |
Mauritius or Réunion ↔ Madagascar | Treatment at nearby Indian Ocean clinics | 1–2 days |
South Africa ↔ Madagascar | Treatment or embryo genetic testing in South Africa | 1–2 days |

Is it safe to transport embryos, sperm and eggs to Madagascar?
Yes, when the shipment is supervised by a specialist courier from start to finish. The risks come from how material is moved, not from the journey itself:
Unaccompanied cargo can sit on a hot tarmac or in a warehouse during a delay, with no one checking the temperature.
Airport X-ray and scanning can expose material to radiation. Our Do Not X-Ray protocol ensures shipments are inspected by hand.
Customs delays are more likely where officials are unfamiliar with reproductive material. Correct paperwork, prepared in advance and in French where needed, prevents most of them.
Temperature failure is prevented by validated −196°C dry shippers with continuous monitoring and a built-in safety margin.

Frequently asked questions
My husband works abroad. Can he freeze his sperm and send it to Madagascar? Yes. He freezes it at a fertility clinic where he lives, your clinic in Antananarivo confirms it will accept it, and our courier hand-carries it at −196°C in time for your treatment.
Are there sperm banks in Madagascar? Donor sperm options in Madagascar are limited. Where a clinic accepts donor sperm, it is usually imported from a bank abroad, which we can source through Greenplace Healthcare and transport for you.
Can I send a fresh semen sample to Madagascar? No. Fresh semen survives only a few hours outside the body, so international transport always uses frozen sperm.
Can I ship sperm out of Madagascar for ICSI or testing abroad? Yes. With a release letter from your Malagasy clinic and an acceptance letter from the clinic abroad, we can export your frozen sperm.
How long can frozen sperm be stored? Sperm kept at −196°C in liquid nitrogen can stay usable for many years. Storage limits are set by each clinic and by the law of the country where it is stored.
How much does it cost to ship embryos to Madagascar? The cost depends on the route, the material and the timing, so we give every client a custom quote. Contact us with your sending and receiving clinics for a clear, fixed price.
Can I carry my embryos to Madagascar myself? We strongly advise against it. Most clinics will not release material to patients, airlines have strict rules on dry shippers, and a single X-ray or customs delay can put your embryos at risk.
I live in France. Can I ship my embryos home to Madagascar? Yes. This is one of the most common routes we manage. We coordinate with your French clinic and your clinic in Antananarivo, and handle the export and import paperwork.
Is PGT (genetic testing of embryos) available in Madagascar? Access to PGT in Madagascar is limited. Many patients send embryo biopsies or embryos abroad for testing, which we can arrange.
Can I freeze my eggs in Madagascar? Some clinics in Antananarivo offer egg freezing. If you froze your eggs abroad, we can bring them home when you are ready to use them.
Is surrogacy legal in Madagascar? Surrogacy is not regulated in Madagascar, and the birth mother is generally treated as the legal mother. Take independent legal advice before considering it.
How far in advance should I book a transport? Ideally two to four weeks, to allow time for clinic letters and permissions. We can sometimes arrange urgent shipments sooner.
Do you handle customs clearance in Madagascar? Yes. Our courier presents all documents in person at customs, and we prepare them in advance with both clinics.
